[OJB-10] -- Changes to create pluggable ProxyFactories. Default JDK is available, as well as a new CGLIB-based implementation that allows for proxies to be generated from classes that do not implement an interface.
This is a backport of a feature added to HEAD.

Updated to XDoclet 1.2.2 and added features to the xdoclet module
* database foreignkeys are now generated for collections
* database foreignkey generation can be switched off per reference/collection using the database-foreignkey attribute
* indirection tables and their foreignkey fields can now be documented using the indirection-table-documentation / foreignkey-documentation / remote-foreignkey-documentation attributes

Simplified and enhanced environment check for building OJB

Restructured samples a bit:
* moved to their own top-level folder, with each tutorial and the misc samples in their own directories
* adjusted build script accordingly
* removed samples and enhance-jdo compilation step from the main build script (is included in the quickstart target)

Extracted tutorial and sample classes from the test directory into their own src sub-directory (including the jdo tutorial)
New target that generates a jar containing the source of the tutorials and other samples
ojb-blank is no longer dependent upon prepare-testdb

* Fixed bug submitted by Bradford Pielech concerning indirection tables for a collection that reference the same class that it is defined in
* Added warning when a field of type java.util.Date without a conversion is encountered
* Updated to XDoclet 1.2.1 (xdoclet and xjavadoc jars are now from the official binary distribution, not generated from source)
